This vendor registration form serves as a prequalification form for all intended vendors, suppliers, and contractors who intend to have a legal and commercial engagement with LES Energy Services.

LES Energy Services will not be held liable for any misinformation or misrepresentation of your organisation based on the information filled.

External service providers are expected to fill the form with the requested information. This is required as part of our procedure for a formal prequalification and registration of our external provider to enable us carry out business transactions.

Please note that the application should be accompanied with;

1. A photocopy of Certificate of Incorporation
2. A photocopy of Memorandum and Articles of Association
3. A photocopy of the Current Tax Clearance Certificate
4. A reference letter from your bank
5. A photocopy of VAT registration Certificate
6. DPR Certificate

Where a formal contractual agreement is required  (or contract renewal) depending on the nature or category of services being rendered, it is expected that we ensure a convenient, flexible and favorable deal is closed for the sole interest of LES Energy Services where a robust risk base analysis is adequately utilized.

All external providers will be evaluated based on their financial capacity, credit facility, site inspections, audit, and other KPI.

Also, a subsequent performance evaluation and re-evaluation of external providers on a 6 Monthly basis will be carried out to advise rooms for improvement, feedback, blacklist or revalidation of services and competency will be made.
